Each practice that participates in the RWJ Partners network is assigned a care coordinator who works with you and your practice to coordinate the care of your high cost and high risk patients. The care coordinator has access to a wealth of resources, including social workers, behavioral health specialists and the resources that are available through Robert Wood Johnson Health System and Rutgers Robert Wood Johnson Medical School.
Practice transformation services and data analytics support are just two other services that RWJ Partners provides. Our Quality Improvement team will work with you to identify any opportunities for growth and development within your practice, and will assist you in achieving your quality metrics. Our data analytics team will work to integrate our system with your Electronic Health Records to extract the data necessary to report quality metrics to our data partners and to provide you with additional data and quality reporting.
Yes, there is also a financial incentive to participate. Both the Medicare Shared Savings Program (MSSP) and Horizon Healthcare Innovations (HHI) programs include shared savings components. If RWJ Partners achieves both cost targets and quality metric targets, you may receive a portion of the savings. Please see How Shared Savings Are Distributed for more information.
